Instructions

1. Follow the steps to request an FMLA ADA Accommodation here: Family Medical Leave Act and Americans with Disabilities Act – Human Resources (opens new page to insideTCC)
2. Once the accommodation request is approved, HR will contact the department to place the equipment order
3. Equipment is located across the District, or an order for new equipment is placed
4. Equipment is delivered to campus, then scheduled for delivery or installation to the user
